Abstract

Objective: To explore the effects of laughter therapy led by clinical nurse specialists on fear of disease progression, resilience and quality of life in ovarian cancer patients, in order to provide intervention measures to alleviate fear of disease progression degree ,improve resilience and quality of life in ovarian cancer patients. Methods: This study was a asynchronous controlled study. Using the convenience sampling method, 100 ovarian cancer patients undergoing radical laparotomy who attended the Department of Gynecology at the First Affiliated Hospital of Army from August 2023 to July 2024 were selected for the study, and the study population was divided into experimental group (n=50) and control group (n=50) according to the order of admission time. The control group received conventional care, and the experimental group received laughter therapy led by clinical nurse specialists for 24 weeks in addition to conventional care. Using two independent sample t-tests, the scores of the Fear of Progression Questionnaire Short Form (FoP-Q- SF), Connor-Davidson Resilience Scale(CD-RISC) and World Health Organization Quality of Life-BREF(WHOQOL-BREF) between the two groups of patients before and after intervention were Compared . Results: There was no significant difference in general information between the two groups, such as demographic data including age, occupation, education level, body mass index, living area, family monthly income,medical insurance payment method, and spouse. Disease-related data included operation duration, operation stage, operation method, total intraoperative access, incidence of postoperative complications, occurrence of severe adverse reactions to chemotherapy, and selection of venous access to chemotherapy.There was no significant difference in FoP-Q-SF, CD-RISC and WHOQOL-BREF scores between the two groups before intervention (all P>0.05). After intervention, the scores of FoP-Q-SF in the experimental group (28.15 ± 4.78) were lower than those in the control group (32.67 ± 5.79), while the the scores of CD-RISC factors in the experimental group were(20.34±3.68, 19.53±2. 64,21.84±2.17, 20.28±3.19), higher than the control group (17.96 ± 2.21、15.78±2.65、17.05±2.23、16.79±2.47) ,the WHOQOL-BREF scores of the experimental groups were(90.24±2.26,90.21±2.43,90.64±2.32,90.68±2.46) higher than those in the control group (77.82±2.65,76.42±2.17,78.71±2.25,77.62±2.18), and the differences were statistically significant(P<0.05) ;The FoP-Q-SF of the experimental group after intervention was lower than that before intervention(32.17±5.84).The scores of CD-RISC factors were higher than those before intervention(13.86±2.07,12.19±1.98,13.75±2.49,11.52±1.86),the WHOQOL BREF scores were higher than those before intervention(62.75±2.43,62.52±2.88,62.58±2.75,62.58±2.75,62.47±2.32). those differences were statistically significant (P<0.05). Conclusion: Laughter therapy led by specialist nurses can effectively alleviate the fear of disease progression in patients with ovarian cancer, promate the recovery of patients" resillence, and improve the quality of life, which is worthy of clinical application.
